There’s a further tragedy in this case, revealed by retired homicide detective Russell Wate. His investigations reveal that parole officers, the police, health services and other agencies were aware of Boulton’s past and deteriorating behaviour – but failures in multi-agency communication and risk management meant they were unable to keep Bethany and DJ safe.
